Have a valid ID
A state issued ID -issued driver's license ~ State-issued identification card -U.S. passport - U.S. military ID ~ State, county and local government IDs - Permanent resident card, or "green card ~ Foreign passport - Driver's license of Mexico or Canada
Fill in all blank spaces of document before I arrive. DO NOT INITIAL OR SIGN DOCUMENT.
Any questions regarding completion of form must be addressed by contacting the person who drafted or requested the notarized document.
